Vasular Tumors in Extremities




Abstract
One hundred and fifty seven cases of vascular tumor including four cases of malignant ones, were treated surgically between january 1980 to December 1991, at the Department of Orthopaedic Surgery in Seoul national University hospital.
Among 157 cases, one hundred and fourteen vascular tumors which were followed up for more than one year, were analysed in terms of pathologic diagnosis and result of the surgical treatment. The duration of follow-up was from 12 to 121 months
(average 21
months). the 'segment', which means topographic area between two major joints such as thigh; leg; foot; arm; forearm; and hand, was used to describe the involved area. If a tumor involved had and forearm, this was considered as 'two segments'
involvement. The results after surgical excision were categorized in four groups; No recurrence, Asymptomatic recurrence, Symptomatic recurrence, and No improvement.
Almost all of the cases were benign tumors(153 of 157 cases), in which hemangioma was the most common (116 of 153 cases). In the 116 hemangiomas, 83 cases were possible to be analyzed. Hemangiomas were frequently forcal in its distribution, and

cases(67%) were excized completely. But 24 cases(29%) were partially excised because of its diffuse involvement. Three cases were treated by repeated embolization. Especially in 15 cases which involved more than two segments, complete excision
was
done
in only one case. These cases resulted in frequent complications such as recurrence, persistent pain or unsightiness. Of the 56 cases which were excised completely, 55 cases(98%) showed No recurrence. But of the 24 cases which were partially
excised,
only 13 cases (54%) showed No recurrence. Eleven cases of glomus tumors were analyzed. All of them showed focal lesions and were excised completely. Eighteen cases of lymphangiomas were analyzed. Sixteen cases(89%) were confined in subcutaneous
layer.
As in hemangiomas, complete resection of tumors were not possible in wide spread cases and the results were similar to those of the hemangiomas. Twelve cases(67%) were excized completely. Six cases(33%) were partially excised Among the completely
excised cases. 11 cases(92%) showed No recurrence and in partially excised cases, 5 cases(83%) showed No fecurrence.
